Your Perceived Wellness Score

How do you perceive your current level of Wellness in these 8 categories?  Print this page and fill it out to find out.

Take 10 minutes or so and rate yourself on a scale of 1-10 (1 is worst, 10 is best) in each of these categories. Then total your score and divide by 80 then multiply by 100. This is your score. Example: if your total was 64 then 64÷80×100=80%

This exercise will help you determine your currently level of wellness and we recommend you set a date 6 months from now to do the exercise again and then compare scores.

Physical Health – What is your physical condition?  Are you receiving good nutrition, drinking plenty of water, getting regular exercise and enjoying the proper weight for your height?

Mental Health – Are you open to new ideas?  Do you seek out new experiences and learn new skills?  What is the quality of the information and entertainment you allow into your mind?

Chemical Health – Are you aware of what chemicals you are putting into your body and how they affect you?  Do you consciously choose foods and nutrients that build you up?  Are you taking drugs or medications that are damaging your liver and your cells?

Financial Health – Are you living within your means?  Is your debt within manageable limits?  Do you make charitable contributions and save for the future?  Are you properly insured?

Family Health – Are you in a loving relationship with shared values?  Do you give your family time and attention?  Do you have a close connection with children, parents and relatives?

Social Health – How well do you interact with others?  Are you able to maintain long-term friendships?  Are you comfortable in new social situations and the company of others?

Career Health – Do you like what you do for a living?  Does your career reflect and advance your deepest values?  Is your work meaningful and suited to your skills and interests?
